[00:00]
Ever fake a smile through a migraine? You're not alone.
[00:04]
Let's talk about pretending you're fine
[00:06]
when you're really not.
[00:10]
Migraine Heroes is a podcast for chronic migraine sufferers
[00:14]
who've seen countless doctors with no results.
[00:18]
Learn how to reduce your migraine attacks
[00:20]
without endless medications or constant sacrifices.
[00:24]
Hello, I'm Diane Ducarme.
[00:26]
I've helped over 500 people break free from migraine,
[00:30]
even after suffering for up to 60 years.
[00:33]
My mission in life?
[00:34]
To bring this relief to a million people
[00:37]
using a unique blend of Eastern medicine
[00:39]
and Western science.
[00:44]
In this episode, you will learn how masking your pain
[00:47]
is a tool for survival in a world
[00:49]
that does not always understand what a migraine is.
[00:53]
How the act of masking, though sometimes unnecessary,
[00:57]
may quietly be intensifying your migraines.
[01:00]
A powerful mindset shift to help you triage
[01:02]
when to mask or not.
[01:05]
Stay with me till the end,
[01:06]
where I'll share a surprising story,
[01:09]
how I've learned, through experience and humility,
[01:12]
not to unmask strangers I sense having a migraine.
[01:15]
Even when the signs are clear,
[01:17]
I've learned that seeing someone's invisible pain
[01:20]
doesn't always mean it's time to speak.
[01:22]
Sometimes, the kindest act is silent recognition.
[01:28]
Masking your pain isn't weakness,
[01:31]
it's a survival strategy.
[01:33]
So many of you have shared this in our onboarding sessions.
[01:37]
How you smile through the meetings,
[01:39]
cook dinner with blurred vision,
[01:41]
or show up for others when your body is screaming for rest.
[01:46]
Because migraines are invisible,
[01:47]
the world often doesn't see them
[01:50]
and instead offers unsolicited advice,
[01:53]
as if a green smoothie, a cup of water, a nap, a walk,
[01:57]
could undo decades of suffering.
[02:00]
After years of being dismissed or told what to do
[02:03]
by people who don't understand,
[02:05]
you learn to say less, to protect your energy,
[02:08]
to quietly endure.
[02:10]
Only a precious few, those closest to you,
[02:12]
can sense the difference between your mask and your truth,
[02:16]
between the days when you're coping
[02:18]
and the days you're crumbling.
[02:20]
This act of faking fine isn't about denial,
[02:24]
it's about navigating a world that rarely poses to ask
[02:27]
what it really feels like to live with migraine.
[02:30]
And here we ask, here we see you, I see you.
[02:35]
Now that I know, I have to confess,
[02:37]
sometimes, I can see it in people,
[02:40]
people I don't even know.
[02:42]
There's a certain look, deadness behind the eyes,
[02:45]
a facial expression, a tension in the jaw,
[02:48]
a way they hold their body
[02:49]
like they're containing something.
[02:51]
I can sometimes guess it.
[02:53]
And a few times, I have whispered,
[02:58]
Would you happen to have a migraine right now?
[03:00]
Maybe to a waitress, to a stranger in a line?
[03:04]
And they looked at me like I have seen a ghost
[03:08]
they know is around and we're not supposed to speak about.
[03:12]
There's a flicker, eyes that come wide open
[03:16]
when they were partially closed, a surprise, an intrigue,
[03:19]
maybe even a relief, but just as quick,
[03:21]
they resume their activity very nervously and very quickly.
[03:25]
I think I've only done it three times,
[03:28]
but I swear I was right each time.
[03:31]
And then I realized my attempt to make them feel seen
[03:35]
may have felt like being exposed.
[03:38]
Me unmasking them didn't offer comfort.
[03:41]
It made them feel unmasked.
[03:43]
And so that is something I no longer do.
[03:47]
I'm extra polite, extra kind, extra smiling.
[03:53]
And there's a reason it is.
[03:55]
Masking has helped you survive, protect your dignity,
[03:59]
and carry on when the world
[04:00]
couldn't hold space for your pain.
[04:03]
And today I want to honor that.
[04:05]
I want to teach you how to use it with intention,
[04:08]
how to mask when you need to,
[04:10]
but also how to recognize the toll it takes
[04:12]
on your nervous system, your relationships,
[04:15]
and your sense of self.
[04:16]
Because constantly hiding your truth
[04:18]
can leave you depleted, disconnected, and misunderstood.
[04:22]
This isn't about abandoning the mask altogether.
[04:26]
It's about finding the middle path,
[04:28]
a place where you can still function in the world,
[04:30]
but also let the right people in.
[04:32]
A place where you can protect yourself and begin to heal.
[04:36]
Let's explore what researchers call emotional labor,
[04:40]
the process of managing your feelings and expressions
[04:43]
to meet social expectations.
[04:45]
This is a real psychological and physiological strain,
[04:48]
especially for migrant sufferers.
[04:51]
A 2020 study in the journal Pain
[04:54]
shows that people with chronic pain
[04:55]
who frequently mask their symptoms
[04:57]
reported higher levels of depression,
[05:00]
anxiety, and migraine frequency.
[05:03]
Because pretending to be okay increases your stress load.
[05:07]
And stress, as we know,
[05:08]
is yet another major migraine trigger.
[05:11]
Then there's the social cost.
[05:13]
A paper in the Journal of Health Psychology by Dr. Smith
[05:17]
found that migraine patients often feel isolated,
[05:21]
invalidated, and ashamed
[05:23]
because their symptoms are invisible.
[05:25]
The invisibility fuels the pressure to act normal,
[05:29]
even when you're crumbling inside.
[05:32]
Another study, Encephalalgia, by Dr. Rains,
[05:35]
shows that emotional suppression
[05:37]
leads to higher cortisol levels
[05:39]
and more intense pain responses.
[05:42]
In short, when you stuff down your feelings
[05:45]
to appear functional, your body suffers.
[05:48]
Masking is not just an emotional toll,
[05:51]
it's a chemical one.
[05:53]
And at the workplace,
[05:55]
a 2022 survey from the American Migraine Foundation
[05:59]
revealed that 68% of people with chronic migraine
[06:02]
hide their symptoms at work
[06:04]
due to the fear of judgment or career consequences.
[06:07]
This creates what psychologists call role strain,
[06:11]
the internal tug-of-war between who you are
[06:14]
and who you are expected to be.
[06:17]
All this pretending consumes an enormous amount of energy.
[06:20]
And when your system is already overloaded with pain,
[06:23]
sensory sensitivity, and brain fog,
[06:26]
there's not much left to give.
[06:28]
No wonder so many migraine sufferers
[06:30]
crash after social events or work days.
[06:33]
Now let's shift to Eastern View for a minute.
[06:36]
In traditional Chinese medicine,
[06:38]
the act of masking pain,
[06:40]
especially emotional or invisible pain, like migraine,
[06:44]
is more than a psychological strain.
[06:46]
It is seen as a disruption of the shen,
[06:50]
the spirit or emotional vitality housed in the heart.
[06:54]
When you suppress your truth,
[06:55]
pretend you're fine,
[06:57]
or constantly push through pain,
[06:59]
it unsettles the shen.
[07:01]
Over time, this can give rise to what we call heart fire,
[07:05]
an over-activation of the heart's yang energy
[07:07]
that leads to mental restlessness, insomnia, anxiety,
[07:12]
and obsessive thoughts.
[07:14]
Heart fire consumes the mind.
[07:16]
It makes your thoughts race,
[07:18]
your sleep shallow,
[07:19]
and your emotional landscape turbulent.
[07:21]
For migraine sufferers,
[07:22]
this can feel like your head is not just in pain,
[07:25]
but caught in a spiral,
[07:26]
looping through fears
[07:28]
to do list overwhelm and emotional pressure.
[07:31]
The heat rises, often quite literally, to the head.
[07:35]
In traditional Chinese medicine,
[07:36]
the head is the meeting point of all yang meridians,
[07:39]
and when heart fire blazes upward unchecked,
[07:42]
it can ignite or worsen a migraine.
[07:46]
So when you mask your pain,
[07:47]
especially repeatedly,
[07:49]
you're not just holding back emotions,
[07:51]
you're allowing heat to build,
[07:53]
unexpressed frustration to simmer,
[07:55]
and your shen to scatter.
[07:57]
The remedy is not always to let it all out,
[08:00]
but rather to find ways to calm the heart,
[08:03]
and gently release what's being held.
[08:06]
From this perspective,
[08:08]
migraines are not just neurological events,
[08:10]
they are a cry from your body,
[08:13]
that your internal harmony has been disturbed.
[08:15]
Holding back tears,
[08:18]
or powering through pain
[08:19]
can create what traditional Chinese medicine calls
[08:22]
liver tissue stagnation,
[08:23]
a condition we've talked about a lot,
[08:25]
directly linked with headache,
[08:30]
there's a similar concept.
[08:32]
When we ignore or suppress our emotions,
[08:34]
we accumulate ama,
[08:36]
undigested emotional toxins.
[08:40]
These build up in the nervous system,
[08:42]
inflaming the mind-body pathways
[08:44]
that migraines exploit.
[08:47]
The hard reality is,
[08:49]
sometimes masking your pain does save your job.
[08:52]
It holds your marriage together during a fragile season.
[08:55]
It keeps a friendship intact
[08:57]
when the weight of your reality
[08:59]
might feel too heavy
[09:00]
to place in someone else's hands.
[09:03]
masking is not deception,
[09:06]
a kind of emotional triage.
[09:08]
You choose silence,
[09:09]
not because your pain is invalid,
[09:11]
but because the risk of loss
[09:13]
feels greater than the burden of hiding.
[09:16]
In traditional Chinese medicine,
[09:17]
this is not denied,
[09:19]
in fact, it's deeply understood.
[09:21]
The heart, which houses the shen,
[09:23]
is not just a beating organ,
[09:25]
it's the seat of your spirit,
[09:26]
your emotional truth,
[09:27]
your capacity to connect.
[09:29]
When you consistently mask,
[09:31]
you may keep the outer world from crumbling,
[09:34]
but the inner cusps builds.
[09:36]
Heart fire begins to rise,
[09:38]
not just from stress or fatigue,
[09:39]
but from holding too much without release.
[09:42]
Your thoughts race,
[09:45]
not always from what you say,
[09:46]
but from what all remains unsaid.
[09:49]
And yet, to unmask too abruptly
[09:51]
can be its own kind of fire.
[09:53]
Traditional Chinese medicine teaches the middle way,
[09:57]
effortless action,
[09:59]
rather than forcing against it.
[10:02]
It's not about always being transparent
[10:03]
or always being guarded,
[10:05]
it's about choosing with wisdom and compassion
[10:08]
when to do one or the other.
[10:10]
Sometimes the most loving thing you can do
[10:12]
is not share your pain in that moment.
[10:14]
Sometimes it's whispering your truth
[10:16]
to someone who has earned your trust.
[10:18]
The eastern answer?
[10:20]
Authentic expression
[10:21]
and radical self-compassion.
[10:23]
Not a rigid rule book,
[10:25]
but a soft philosophy,
[10:27]
to gently release what you carry when you can,
[10:29]
and to forgive yourself when you can't.
[10:32]
Your migraine is not just malfunction,
[10:35]
and part of your healing is learning when to speak,
[10:40]
without burning your spirit in the process.
[10:42]
This is a very difficult topic,
[10:44]
and I hope you do find your own balance.
[10:47]
I also hope you do push our door at Migraine Heroes
[10:50]
to no longer have to make those very hard trade-offs.
[10:55]
every small step you take
[10:57]
moves you closer to freedom.
[11:00]
you are resilient,
[11:02]
and you are no longer alone.
[11:04]
Keep rising, Migraine Hero.
